Remote Patient Monitoring for Chronic and Complex Care Needs

This jurisdictional scan of Canadian health systems and selected international jurisdictions aims to identify implementation strategies for integration of remote patient monitoring into existing models of care for individuals with chronic disease or complex care challenges.

Newfoundland and Labrador has a geographically dispersed population that presents a unique challenge in service delivery across the health system. In an effort to improve patient outcomes and continuity of care for populations in rural and remote areas, the Government of Newfoundland and Labrador plans to expand the use of remote patient monitoring technology across the province. Information gathered from this jurisdictional scan will inform the government’s strategies to effectively implement and evaluate remote patient monitoring models in the province.
